Licensed Practical Nurse (LPN)



Assists in examination and treatment of patients under direction of medical and behavioral health providers in an outpatient clinic setting. Performs duties according to scope of practice.

Essential Duties and Responsibilities

•Preparing and rooming the patient for exam by obtaining vital signs and gathering/documenting/updating pertinent health information (i.e., chief complaint, allergies, and/or medications).

•Assisting the provider with procedures and surgeries such as pelvic exams, EKG, colposcopy, etc.

•May perform basic and advanced clinical support tasks or skills based on the specialty and appropriate advanced skills and competencies such as but not limited to removal of sutures and staples, straight catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen), phlebotomy – butterfly, venous heel stick, EKG, spirometry, etc., under clinical supervision.

•Provides translation for Providers as needed and able.

•Administering routine medications, under providers orders, which may include but are not limited to immunizations, antibiotics, vitamins, and topical agents.

•Completes all necessary forms or paperwork.

•Providing basic patient instructions on the performance of routine tasks or skills. Following through with necessary procedure or test requests, pre-approved care algorithms, new appointment times and referrals to other facilities or services. Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.

•Communicating with physicians and other members of the health care team to ensure smooth clinic flow and adjusts as necessary. Effectively communicating accurate and timely information with the patient.

•Answers telephones politely, evaluating and responding to patient problems and questions and relays information to the provider.

•Performing laboratory procedures (i.e., strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions/infection control guidelines.

•Follows all facility, state and federal policies and procedures.

•Assist providers in timely completion of medication refills.

•Regular and Reliable Attendance.

•Other duties as assigned.

Min. Qualifications

Proficient computer skills including keyboarding, navigation within a windows operating system, use of electronic mail and electronic medical records systems. Excellent communication (written and verbal) and interpersonal skills. Ability to develop rapport and maintain positive relationships with a variety of patients, family, staff, and physicians. Good organization, prioritization, and problem-solving skills. Ability to multi-task with frequent interruptions.

Education/Experience Requirements

•High School Diploma or equivalent required.

•Licensed Practical Nurse (LPN) license issued by the state of Wisconsin.

•Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) required at time of hire.
